bacha
One very important point: you cannot include sexual services in any contract as it will make the agreement unenforceable in any US state. 'Meretricious consideration' is verboten. In California in particular, a contract between unmarried persons is invalid "if sexual acts form an inseparable part of the consideration for the agreement." Marvin v. Marvin, 18 Cal. 3d 660,672 (1976).

bacha
so mostly you want to write down what you've agreed to to have a record of it and a common reference point but obviously most topics would render any contract unenforceable never mind that you most likely would not want it to end up in court. and if introduced into court, such an agreement might prove very harmful against you.
